To assist Individuals and Healthcare Providers integrate Holistic, Non Drug Modalities and Practices into their Wellness Protocol in an effort to provide long term healthy lifestyle support, including the client’s ability to advocate for their own wellness.
Our premise is based on a truly Holistic approach, embodying Mind, Body, and Spirit methodologies to empower clients and make a difference in epidemic addiction, depression, anxiety and suicide rates in our population.

We offer Strategic planning to help successfully transition clients from in-patient and out-patient programs. Our focus is to help clients take control of their own health and wellness utilizing holistic tools, support and resources. Using a Holistic approach which addresses the Body, Mind and Spirit, empowers a richer Client engagement and motivation to reach achievable goals.
Improved mental and physical health and happiness in the individual is greatly affected by caring interaction and feelings of connection. We personalize a transitional plan that supports individuals to stay mindful and focused on achieving their best outcomes while integrating holistic modalities and resources into their daily lives.
We know that change is often difficult to embrace; especially when one is recovering from injuries, pain (emotional as well as physical), addictions or a combination of issues. Our goal is to offer meaningful follow up encompassing truly healthy holistic habits, belonging to an inspirational and caring community and navigating how to live one’s best life with tools and support.
PROCESS
Much like a Life Coach, helping clients transition and integrate new habits, modalities and healthy lifestyle changes involves a process that takes time and commitment. We go through the following Steps in order to help successfully design and implement a Transition Plan.

Gather Information.
We inqiure about overall Health history - physical, mental and family
We review Treatment. Experience and Results, including possible dialog with practitioners
We host Personal Client Interviews and discussions regarding different modalities effectiveness, what did and did not work, and why; current daily activities including work activities and what a client does for fun; community dynamics with family and friends, support resources in place, time management, client goals, financial status and future plans.

Create Transition Plan
We use the information collected in Step 1, along with Client Assessment information in Step 2, to develop a Transition Plan for clients that keeps them moving towards their goals. Plans encompass hyper-personalized activities, tools and modalities taking into account client’s affinity for program and providing stable resources to help execute.
Our Plan will outline areas to focus on based on desired Goals and Needs of client; it will take into consideration past success and predictable likelihoods to design a program that will best engage and support clients with meaningful outcomes. Communication with client will be implemented with a variety of methods, including by personal meeting, FaceTime visits, text, email and phone to ensure engagement is easy, frequents, comfortable and convenient.
Plans are designed with custom time frames for implementation depending on needs of our clients. We require at least a 90 day roadmap and highly recommend a 1 year Transition Plan. Scheduled in-person follow-ups are required after activating each Module of Focus.

How to set meaningful goals to live a better more fulfilling life.
Integrating holistic practices into your daily life and time management.
How to be your own best advocate for your life, your health, and your happiness.
Tracking and assessing your progress in order to definitively keep your holistic plan relevant and effective as you heal.
Regular evaluation and modification as needed of your personal plan in order to best maintain desired results and goals.
